Lifeline Assistance Program FAQ
How do I know if I am eligible?
Individuals over the age of 18 are eligible to participate in the Lifeline program if they (or a child or dependent) participate in one (or more) of the following programs:
• Supplemental Nutrition Assistance Program (SNAP)
• Supplemental Social Security (SSI)
• Federal Public Housing Assistance (NPHA)
• Veterans Pension and Survivors Benefit
Individuals are also eligible if they live in a household with income less than 135% of the Federal Poverty Guidelines. The guideline is based on your household size and state.
Are there any restrictions?
The discounts available in the Bark Mobile Lifeline program are funded by the Federal Government and is a government assistance program. The Lifeline program is limited to one benefit per household, consisting of either wireline or wireless service – but not both. Only eligible consumers may enroll in the program. Lifeline service is non-transferable. Documentation of eligibility is required for enrollment.
Customers may purchase additional services available to a non-Lifeline customer. Customers must establish phone service prior to applying for the Lifeline discount. The name on the phone bill must match the name of the household member participating on the eligible program.
How much will qualified customers pay?

- Unlimited Nationwide Calling
- Unlimited Nationwide SMS & MMS
*Taxes and fees are included. Roaming data will be at 3G speeds.
Extra data may be purchased for $10 per GB.
How do I apply?
To apply for Lifeline service individuals may visit any Bark Mobile Retail store.
What proof of eligibility do I need to provide? Proof of eligibility is required by submitting an application and certification form signed under penalty of perjury, and by providing a copy of any dated document which verifies participation in one of the qualifying programs. Proof of total household income may be required for income based qualification. Lifeline benefits will take effect when proof of eligibility is verified.
